Novolink typically serves high-complexity patients with multiple chronic conditions who have had 2+ medical admissions in the last 45 days or 3+ hospital events in the previous 60 days related to their chronic condition(s). Your healthcare provider or insurance company can help determine if you qualify.

How is Novolink different from traditional home health care?

Unlike traditional home health, Novolink offers continuous, 24/7 monitoring with FDA-approved technology, specialist-led care teams, and a focus on the most complex patients. We aim to help patients not just go home but stay home by breaking the cycle of frequent hospitalizations.

What kind of technology does Novolink use?

We use advanced, FDA-approved monitoring devices that can track vital signs, perform ECGs, conduct spirometry tests, and more. Our technology is programmed to each patient's baseline and integrated with our EMR system for rapid response to any changes.

Who will be providing my care?

Your care will be managed by a multidisciplinary team led by specialist physicians. This team may include nurse practitioners, behavioral health specialists, social workers, and other healthcare professionals as needed for your specific condition.

Is Novolink covered by insurance?

Novolink works with various insurance providers, including Medicare Advantage plans. Coverage can vary, so we recommend checking with your insurance provider. For Humana patients, we have a specific pilot program running through December 2024.

How long does the Novolink program last?

The duration of care varies based on individual patient needs. We typically start with a 60-day program, after which we review progress with your primary care physician or managed service organization to determine if continued care is beneficial.

Can I still see my regular doctor while in the Novolink program?

Yes, we work in coordination with your primary care physician and specialists. We provide regular updates to your doctors and ensure continuity of care.

What support do you provide for caregivers?
