Patient Eligibility Criteria
Our minimum standards for patient eligibility include:
- Patients with multiple or advanced chronic conditions
- History of frequent hospitalizations (e.g., 2+ medical admissions in the last 45 days or 3+ hospital events in the last 60 days related to chronic condition(s))
- Patients who are stable enough to go home but have struggled to stay home
These criteria are provided to PCPs, Hospitalists, and other collaborating specialists to assist in initial patient screening. While they serve as a starting point, our Novolink clinical team conducts a thorough, collaborative chart review for each potential patient. This ensures we can safely and precisely deliver the care needed for each individual.
