Eligibility Criteria

Patients who may benefit from Novolink's program typically meet the following criteria:

• 2+ medical admissions in the last 45 days OR 3+ hospital events in the last 60 days related to chronic condition(s)
• Polychronic and/or advanced chronic conditions (e.g., Class 4 heart failure, COPD)
• Patients who cycle between home and hospital
• Stable enough to go home but not to stay home
• Most have 12-month costs over $150,000
